TUITION/PAYMENT AGREEMENT
The undersigned, parents or legal guardians of __________________________________________,
jointly and severally promise to pay to the order of St. Barbara School, the sum of $________________
as and for tuition for
the 2008-09 school year. Said sum shall be payable as follows:
10 Equal payments payable on the first day of each month beginning
August 15, 2009 through May 15, 2010.
or 12 Equal payments payable on the first day of each month
beginning July 1, 2009 through June 1, 2010.
or Payment in full by September 15th, 2009 with 5% discount given.
(Please indicate which option you choose to pay your tuition for the 2008-09 school year.)

4 days a week is $140 per month x 9 = $1260.00
